Medical records/bills can be mailed, faxed, or emailed in a Zip file.

Electronic documents need to be in one of following formats: .rtf, .doc, or.pdf

Video needs to be in the following format: 8mm cassette or mpeg, mpgII, and .wmv for computer files.

Pictures need to be in one of the following formats: .gif, .jpg, .JPG, .png
